Prostaglandins, Period Pain & Why the Fats You Eat Matter
If you've ever wondered why your period can leave you feeling like your uterus is staging a full-blown protest, we need to talk about prostaglandins.
They're tiny molecules, but they have a surprisingly big job in the body—and they may have more to do with your period pain, exercise recovery, digestion, and even inflammation than you realize.
Let's break it down.
First: What exactly are prostaglandins?
Think of prostaglandins as tiny text messages your body sends between cells.
Your body makes prostaglandins from fatty acids—the building blocks found in fats. Once they're made, they help tell different tissues and cells what to do.
Their messages might sound something like:
“Contract.”
“Relax.”
“Increase blood flow.”
“Start an inflammatory response.”
“Help resolve inflammation.”
“Protect this tissue.”
“Move the digestive tract.”
So prostaglandins aren't inherently “good” or “bad.”
In fact, we need them.
One of their most important jobs is helping regulate inflammation—and here's the part that often gets misunderstood:
We actually NEED inflammation.
Inflammation isn't automatically a bad thing.
When you work out, for example, your body creates a temporary inflammatory response. That's part of how your body responds to the stress of exercise, repairs tissue, and adapts so you can become stronger.
When you get injured, inflammation helps your body respond to the damage.
The goal isn't to eliminate inflammation altogether.
The goal is to have an appropriate inflammatory response—and then be able to regulate and resolve it.
So what does this have to do with your period?
A lot. ☠️
During your period, certain prostaglandins—particularly PGE2 and PGF2α—increase in the uterus. One of their jobs is to help your uterus contract, which is necessary to help shed the uterine lining.
The problem?
When prostaglandin activity is higher, those uterine contractions can become stronger and are associated with more inflammation and pain...In simplified terms:
More prostaglandin activity → stronger uterine contractions + more inflammatory signaling → potentially more period pain.
This is also one reason medications like ibuprofen can help with menstrual cramps. They inhibit enzymes involved in prostaglandin production, which can reduce prostaglandin levels and, in turn, reduce pain and inflammation.

What you eat provides some of the raw materials your body uses to make these signaling molecules.
One of those raw materials is arachidonic acid, an omega-6 fatty acid. And before you hear “omega-6” and immediately think INFLAMMATION = BAD, let's pause.
It's not that simple.
Arachidonic acid isn't inherently bad or something you need to eliminate. Your body needs it for normal functions, and it can be converted into a variety of signaling molecules involved in inflammation, immunity, blood flow, reproduction, and more.
The important part is that your body has multiple pathways it can use to create different signaling molecules.
Other fatty acids—including omega-3 fatty acids like EPA and DHA—can also be converted into signaling molecules that help regulate and resolve inflammatory responses.
So the conversation isn't:
Omega-6 = bad
Omega-3 = good
It's much more nuanced than that.
Think of it like your kitchen.
Your dietary fats provide some of the ingredients.
Your enzymes are the chefs.
And prostaglandins and other lipid mediators are the finished dishes.
Different ingredients can be used to make different signaling molecules, and those molecules can have very different jobs.
Some help start an inflammatory response when your body needs one.
Others help regulate blood flow, protect tissues, or resolve inflammation once the response has done its job.
Your body needs the ability to do both. That's why I don't love the idea of labeling foods—or entire categories of fats—as simply “inflammatory” or “anti-inflammatory.”
The human body is much more sophisticated than that.
The pathway, simplified:
Fatty acids
↓
Enzymes
↓
Prostaglandins + other lipid mediators
↓
Messages to your cells and tissues
And those messages can influence things like: Inflammation. Blood flow. Digestion. Reproduction. Pain. Immune function. Tissue repair.
Pretty cool, right?
What does this have to do with your everyday life?
Quite a bit.
Your period
Prostaglandins help your uterus contract. Higher prostaglandin activity is associated with stronger uterine contractions and menstrual cramps.
Exercise
Exercise creates a temporary inflammatory response. That's not necessarily a bad thing—it's part of the process of repairing and adapting to the stress you've placed on your body.
Prostaglandins participate in these local responses.
Digestion
Prostaglandins help regulate movement through your digestive tract and contribute to protecting the lining of your stomach.
Injury & healing
When you get injured, prostaglandins participate in the inflammatory response that helps your body recognize and respond to the damage.
Blood flow
Prostaglandins help regulate how blood vessels behave, influencing circulation and blood pressure.
So should you be afraid of inflammation?
We aren't afraid of inflammation - but rather intrigued by it. You want your body to be able to turn inflammation on when it's needed, regulate it appropriately, and turn it back down when the job is done.
And your diet is one piece of that bigger picture.
The fats you eat don't simply go straight into your body and “cause inflammation.” Instead, they provide some of the raw materials your body uses to create the signaling molecules that influence inflammatory and other cellular responses.
So rather than asking: “Are fats inflammatory or anti-inflammatory?”
A better question might be: “What types of fats am I regularly eating, and how do they fit into the bigger picture of inflammation regulation?”
Because nutrition isn't about finding one villainous food or one magical “anti-inflammatory” ingredient. It's about giving your body the raw materials, nutrients, and variety it needs to do what it was designed to do.
And yes—that includes knowing that the humble fat on your plate can ultimately help influence some very important messages happening inside your body.
